Northwood University Athletics Sports Medicine

Dear Student-Athlete and Parent: 

On behalf of the Athletic Training staff, I would like to welcome you to Northwood University.  This email will explain the medical clearance process and paperwork that must be completed.  All required paperwork and a pre-participation physical need to be completed before you will be allowed to participate in athletics. 

Northwood University’s Athletic Training Department will be using SportsWare Online (a secure medical database) for the collection of medical information and all injury tracking.  This database will allow you, the student-athlete or parent, to fill out all required information online and attach any other required forms (Pre-participation physical and insurance card) that cannot be signed electronically.  We hope this will make gathering medical information more efficient, allowing for better care of the student-athlete. 

Pre-Participation Physical:

You must have a pre-participation physical completed BY A PHYSICIAN prior to participating in any activities at Northwood University.  This includes practices, scrimmages, and weight-lifting sessions.  A Pre-Participation physical must be completed after February 10th, 2023 (6 months prior to your report date).  You can attach a completed Pre-Participation physical on SportsWare, email (athletictraining@northwood.edu), or fax (989-794-6189) a copy.

SportsWare Online:

You will also be required to fill out medical information including general information, address, emergency contact information, insurance information, medical alerts, and medical history.  There will also be four (4) required forms that will need to be filled out and electronically signed in the “Forms” section in SportsWare Online.  Login instructions are provided in an attachment to this at the top of this page.

Sickle Cell Trait Status:

New this year, the NCAA is requiring that all student-athletes provide documentation of their Sickle Cell Trait (SCT) Status to the school’s medical staff.  You will NOT be cleared for activity at Northwood until we get the results of your test.

Student Insurance Waiver:

All international students are automatically enrolled.    All MBA or Graduate students must also do a waiver.
